What type of dwelling do you live in?  (Home, Apartment, Townhouse) Do you own or rent?  If you rent, please include Landlords name and telephone number so we can verify consent for adoption.   Also include size and type of fence or kennel area.

 

What other pet’s do you keep?  Include type of pet, age of pet, if they are spayed/neutered and current on vaccinations.

Have you ever given a pet to a shelter or rescue group?  What were the circumstances and if they were repeated would you make the same decision?

 

What will you do if the animal has issues such as barking, digging or housebreaking?  What type of discipline will you use?  How long do you anticipate it should take for this animal to transition into your home?
