LUCKY DOG ADOPTION POLICY & CONTRACT
- Minimum donation of $125.00 to offset medical expenses. Dogs are spayed/neutered, have all of their vaccinations, have been heartworm
tested and completed the Lucky Dog training course (dogs will not be released prior to completion of training.)
- All applicants must fill out an application form. All things being equal, adopters will be considered in order of receipt of the applications.
Forms are available at the Hutchinson Animal Shelter or at rlvshollowell@hughes.net (Our dogs are identified with their name/Lucky Dog on the website.
Information is available about the dog by clicking on their name.)
- Decision for placement will be made by the Adoption Committee. Placement will be based on:
a. An interview with the prospective family
b. Personality compatibility between the dog and family
- After adoption, health, (vaccinations, heartworm preventive, and necessary veterinary care), welfare and continued training of ___________becomes my responsibility and adoption releases the Friends of Animals in Need, Inc. and the Hutchinson Correctional Facility from further responsibility or liability for any illness, injury or damage which _________________ may cause to any person or property. I UNDERSTAND THE POTENTIAL FOR INJURIES TO FAMILY, FRIENDS OR OTHER PETS WHEN BRINGING A NEW DOG INTO THE HOME. I RELEASE FRIENDS OF ANIMALS IN NEED, INC. FROM ANY LIABILITY SHOULD INJURIES OCCUR.
- In the event I can no longer keep_______________, I agree to notify Friends of Animals in Need, Inc. immediately. ("Friends has the option of returning to the dog to the Lucky Dog program.)
- I agree that as a companion dog___________will be indoors except to eliminate, exercise and play. If I violate this agreement, I give permission to the Lucy Dog representatives to reclaim __________________.
